Influenza Waiver Completion and Compliance Instructions
Students who choose to submit an influenza waiver must complete the waiver form in full and submit it by their compliance deadline. Failure to submit the completed waiver by the deadline may result in administrative cancellation from the program.

Open the attached PDF and review all sections of the document carefully.
Complete All Required Fields
Provide all requested information, including signatures, dates, and any additional acknowledgments noted on the form.
Follow All Instructions Listed on the Form
Ensure you meet any outlined requirements and complete each step exactly as described.
Verify Submission
After submission, Axon staff will confirm that your document has been submitted appropriately and will notify you of their approval decision once determined.
Important Reminder
Missing your compliance deadline—whether for submitting your influenza vaccination or submitting the influenza waiver—may result in administrative cancellation from your program. Please plan accordingly.
