While on your shift, you must log your patient notes in the Axon Student Portal for that specific shift. These entries must be completed before completing the check out process. Only patients documented in the portal will be eligible for entry in FISDAP—for example, if you log two patients in the portal, you’ll only be able to document and receive credit for the same two patients in FISDAP.
You're welcome to take additional paper notes during your shift if that helps you write stronger narratives later—use whatever method works best for you. However, the notes entered into the portal are mandatory and serve as the minimum required documentation for each patient encounter. Here is a helpful resource to guide you through the process:
