End of Clinical/Field Shift Evaluation Requirements
Once you begin the checkout process at the end of your clinical or field shift (within the last 10-15 minutes), you must complete all required evaluations before leaving the site, and your preceptor should be with you during this process.

Before you start, read the instructions on your checkout page carefully to ensure you understand each step.
Start the Checkout Process:
The evaluation process takes about 5 minutes
from start to finish.
When prompted, please be sure to enter your preceptor’s contact information (full name, email, and phone number) directly in the Axon Student Portal in case Axon needs to reach out for any reason. A piece of paper or an email with this information will not be accepted. Note: Preceptors may decline to provide this information.
Preceptor Signature:
The system will first prompt you to have your preceptor sign the shift summary using your device. This is simply confirming that the student attended and completed the entire shift. In the rare case that your preceptor is unavailable, please review the knowledge base article related to an alternate signer. If your preceptor is available,
DO NOT click the checkbox (and do not allow your preceptor to click the checkbox) that says “My preceptor is not available.” Selecting this option will prevent your preceptor from completing your evaluation and may result in your shift not receiving credit. Alternate signers are only acceptable in
true emergencies where your preceptor is unavailable. If you use an alternate signer, you must email
support@axoneducation.com immediately so our faculty can determine whether your shift is eligible for credit.
Preceptor Evaluation:
After signing, your preceptor will complete their evaluation of you directly on your device.
Make sure they answer all questions before submitting.
Once done, they’ll return the device to you.
Student Evaluation:
You will then complete your own evaluation, which includes rating:
The site you attended
Your preceptor
Your own performance
Submit and Confirm:
Once you click Submit, you’ll see a confirmation message that your checkout has been completed. At that point, you’re cleared to leave the site.
