How to Obtain the Required Preceptor Signatures/Evaluations 

To ensure your shifts are counted, you must complete all end-of-shift requirements before leaving the site. These requirements are essential and non-negotiable. If you experience unforeseen circumstances, like Wi-Fi issues or website maintenance, you can use the attached packet for preceptor evaluations and signatures. If these requirements aren’t completed at the end of your shift, you won’t be able to retrieve them later and will need to replace the shift.

Note: Do not use the FISDAP mobile app for obtaining signatures or evaluations. A laptop or device with desktop access is required for completing these tasks, or you can print the packet if you’re unable to bring a laptop. 

 

Obtaining Preceptor Evaluations:

  1. Click into the appropriate shift for their clinical/field experience date.

  2. Scroll to the bottom until they reach a dropdown menu under the "Shift Evaluations" section

  3. Choose a form from the dropdown menu and click "Go"

  4. Complete one evaluation at a time.

  5. Once a student or preceptor presses "Save", go back to the dropdown menu and choose another form until each of the required evaluations are completed by the appropriate individual.

 

Obtaining Preceptor Signature:

  1. A student DOES NOT click into their current shift that they have created.

  2. A student should see their shift list (a list of all attended and/or scheduled clinical/field shift dates)

  3. Student clicks on the bubble under "Preceptor Signoff" for the appropriate clinical/field experience date.

  4. In the preceptor sign-off box, the preceptor will type their full name and then sign in the box below. Please make sure it is an actual signature and not just an "X" or something like that.

  5. Make sure your preceptor ONLY clicks "Sign Off".

    • If the preceptor clicks the box that says "Lock patient care/narrative documentation for this shift", students will be unable to continue updating their PCRs. Therefore, if you're completely done, they can check that box, but if a student still has narratives to complete, the preceptor should only sign off, leave the box unchecked, and the student can lock the shift once they complete documentation within 48 hours of shift end time.

  6. Preceptors are encouraged to make notes in the "Shift Summary " and "Plan of Action " sections, but it is not required.
