How Do I Check-Out of My Clinical/Field Shift?

At the end of your shift, you must complete the check-out process in the Axon Student Portal before leaving the site. This step must be done in real time, as your preceptor is required to complete part of the process. Do not leave your shift location and attempt to complete check-out later. Here is a helpful resource to guide you through the process: 


Failure to properly check out may result in the shift being disqualified from counting toward your required hours/patient contacts. Additionally, checking out too early or too late can impact the hours awarded for that shift.

In the event of a technology issue, students should always come prepared with printed copies of the evaluation forms so that preceptors can complete them by hand if needed. If you encounter any issues during check-out, you must contact the Student Support Team immediately for assistance.
